New Patient Registration
For first-time patients. If you have been seen here in the past 24 months, your information is already on file.

Collision details, insurance information and the parties involved. Bring this if you were in an accident.
Open PDFOnline submission of these forms is being set up; a destination inbox is pending confirmation from the clinic. For now, print and bring them with you, or arrive a few minutes early and complete them at the desk.
Payment
Self-pay, transparent, no surprises
First Health is a self-pay medical practice, which keeps our pricing transparent and our care focused entirely on you. We do not accept medical insurance. Accepted at the time of service:
- Cash
- Debit cards
- Credit cards (3.5% processing fee applies)

No health insurance? You can still be treated today.
If you were hurt in a collision, the at-fault party's auto insurance is responsible for your medical bills. We can also treat on a lien, meaning your care is paid out of your settlement, not out of your pocket while you recover. Our team handles the paperwork the insurance carriers require.
