New Patient Registration
For first-time patients. If you have been seen here in the past 24 months, your information is already on file.
Motor Vehicle Accident Form
Collision details, coverage information and the parties involved. Complete this if you were in an accident.
Health History Form
Past and current conditions, medications and allergies.
Online forms are sent straight to our front desk. Please share only what the form asks for, and bring your photo ID to the visit.

Self-pay, transparent, no surprises
First Health is a self-pay medical practice, which keeps our pricing transparent and our care focused entirely on you. Accepted at the time of service:

- Debit cards
- Credit cards (3.5% processing fee applies)

Flexible ways to pay for accident care
Cash pay options are available. Many auto policies include medical payment coverage, and if another driver is at fault, their carrier may cover your care. If your case is being handled by an attorney, attorney liens are accepted.
